Establishing Care With a New Provider: My Journey

A friend of mine recently moved to a different state and is facing the daunting task of finding and establishing care with a new healthcare provider. When she shared this with me, I remembered all of the feelings and emotions that had flooded me, when I faced the same thing. Sharing my journey to help others is a big reason why I started this blog, so it only seemed right to sit down and write.

This will be a multi-part post. I'll share my story, and then I'll lay out some guidelines to use in your own journey (First Step and Step by Step).

I’ve been there. It can be a time of mixed emotions. Anxious. Overwhelmed. Scared. Exciting. Hopeful. 

I’d known for a long time that my doctor was out of treatment options for me to try. He did his best to help me maintain, but he had nothing new in his toolkit to offer. Despite him outright telling me multiple times over several years that this was the case, I wasn’t able to hear and accept what he was telling me. 

Honestly, I was afraid. I’d been with this doctor for 7.5 years. He was the first headache specialist I was referred to in the year following my car accident (The Battle Begins). He had helped me through so much, and I was comfortable with him.
  • I was afraid of starting with someone new. 
  • I was afraid that I’d have to retry failed treatment options. 
  • I was afraid I’d have to prove and defend my pain and disability levels. 
  • I was afraid of the monumental task that starting with a new doctor while having such a complex medical history felt like.
I had spoken with a couple doctors at patient conferences about what was important for establishing a new relationship with a healthcare provider. This helped ease my mind a little bit about what I needed to do to prepare, but it still took me a year or two to get to a place where I was prepared to take that step. 

My doctor was very supportive and kind about me moving on to a different headache specialist. He sent the referral and continued my care until I could get in with the new doctor. 

I prepared a lot at home for that new doctor, but much of it was to make sure my mind was in order... for me, having order can help ease my anxiety, even if only a little bit. 

My first appointment with my new headache specialist was incredibly smooth. I had submitted all of the new patient paperwork prior to the appointment. I never felt like I had to defend myself in any way. He didn’t question how much the pain has disrupted my life... he trusted my word. That was hugely uplifting and powerful. Since each doctor has different preferences about what they want tracked, I clarified that with him on our first visit. We also discussed expectations for communication between visits and for emergencies. 

I left that visit “grateful for renewed hope with a new specialist. Looking forward to this chapter, as it already promises opportunities to make positive changes in my life. The road won’t always be pleasant… sometimes it’ll be downright miserable… but, I have a good support team that knows I can do it, even when I’m uncertain.” 

I only saw this doctor a few times before I moved a couple states away. I struggled with separating from a doctor that was a true treatment partner. We had barely scratched the surface, and the fear that I may not find another doctor that I trusted and would be a partner in my care lay heavy on me. 

Time to start over... but on a much larger scale, as I would need a new dentist, eye doctor, and primary care doctor, in addition to a headache specialist. It felt like a heavy task, but it was no longer monumental. There was a sense of hope that getting new perspectives of my health might reveal new possibilities in treatment and quality of life. 

I chose to find a headache specialist first. My previous doctor referred me to my current doctor and continued care until I could be seen. 

My first appointment with my current headache specialist was a lot like the previous one. I submitted all of the new patient paperwork prior to the appointment. The appointment was smooth, which I attribute partially to me continuing to hone in on my experience. We, too, discussed expectations and ensured we were on the same page as one another. 

That appointment initiated a series of consults and testing to address other health concerns and comorbid diseases. I saw around 8 new doctors (in addition to testing) in the following 5 months. Each doctor needed my story, but from a different vantage point... from the vantage point of his/her specialty area. It was exhausting, but I learned so much. 

I realize that not everyone has access to a headache specialist and that not everyone will have a smooth, positive experience. There are not even close to enough headache specialists available (there are less than 500 in the United States), and most doctors don't receive much education about headache medicine. All we can do is be prepared to do our best to find a healthcare provider (even if it's not a headache specialist) that is willing to partner with us in our care.

Moving-In

Well, we finally moved all of our stuff down from Tulsa to Texas. Yesterday was a LONG day! Luckily, we had plenty of help and support from family and friends (we appreciate it sooo much!). We were able to load everything up in the morning, make the drive (took about 6 hours), and unload everything into our new apartment. I was even able to get some things unpacked in the kitchen (thank you, Mom!). We're both so excited to have our stuff down here now, but we're still staying with family through Jeremy's work-week (Thursday through Sunday).

I know that it's still going to take PLENTY of time to get unpacked and settled in, BUT I'm so glad that things are moving right along. After moving everything to the apartment (we still have some stuff here... staying with family... but the majority is moved), we can unpack and settle it, whenever it's good timing for us. I have to pace myself so much (which I hate!); but it's much easier to take breaks and stuff, when I'm on my own time.

BONUS:  Jeremy and I met our downstairs neighbor yesterday evening. We know more about them (i.e., the woman's name, and that the man is a UT fan) than we knew about any of our neighbors in Tulsa!

Well, we're both EXHAUSTED! My body was NOT happy yesterday, and I'm still feeling it today (as expected, though). But, I didn't have to take any of my abortive meds either day, which is great! It's going to take a while to make things up to my body, but it's GREAT to be back in Texas!!!

An Update for a Rough Week

I've had a very rough week. I started having some new, strange symptoms, after I ate dinner Monday night. I started having severe pain in my right shoulder, chest, and back... the pain was so severe that I had trouble moving, breathing, coughing, etc...

While it improved some, the following day was still very painful. It has slowly gotten better, but it's still painful.

I'm also having pain just below the sternum, and often have severe abdominal pain. I found out in January 2009 that I have a small hiatal hernia and gastritis. So, I called my GI doctor, but he can't see me until the end of August.

I set up an appointment with my primary care physician for Thursday, but the pain was still so bad that I moved the appointment up, so I saw him on Wednesday. We did an EKG to make sure my heart is okay (with acute chest pain, pain and numbness in my arm, etc...). It came back fine... thank God!

We also took some blood to check my kidneys, liver, etc... Everything came back normal (kidneys, liver, electrolytes, and all of the stuff checked in the comprehensive metabolic panel). By the way, I have to brag on myself a little. Even though I'm still losing weight, my calcium and protein (I mention these because I was deficient in them both, when I was in the ER last September) are both very good. :-) 

I had a gall bladder ultrasound yesterday. My doctor called late that afternoon and said that the results are normal. So, I'm supposed to continue taking the daily Omeprazole that he started me on the other day, and monitor the pain.

I think what jump-started all of this mess (we believe it's a combination of the hiatal hernia and gastritis) is all of the stress lately (moving, husband getting a job, temporarily living with his parents, getting used to everything here, etc...), as well as my continuing weight-loss.
I was 120lbs at the time of my accident (October 2008), and have been up and down (mostly down) since then. I was steady at 110 lbs (the end of January through mid-March), then I was down to 105.5lbs by the end of April, we stopped the Topamax beginning/middle of May, but I just keep losing weight... At the end of June, I was 104.5 lbs, and the first symptoms of the hernia pain were the day after that weighing. The horrible hernia pain started July 11, and I weighed 102.5lbs on the 13th.
I've been taking so many medications over the past few years, that it's just tearing my body up (and NOT helping the things they were meant to treat!). I really wish I didn't have to deal with ANOTHER source of PAIN. I just don't have the strength right now. 

I think that once we get moved and everything starts to level off, that things will begin improving a bit. I really need to figure out how to gain some weight because I think that's contributing to A LOT of the new symptoms, as well as worsening old ones. I just don't know how to eat more / gain the weight... it seems an impossible task with all of the chest/shoulder pain + migraine pain + nausea...

On a POSITIVE note, I went to my neurostimulator doctor this week. I got my stimulator rebalanced/reprogrammed (and added a few more programs), which should help. My doctor wants to see me every 2-3 weeks (now that I'm living closer), in order to try to get the stimulator helping me more. He hasn't given up yet. :-) I really hope we figure out a way to better manage the migraines.

Apartment Hunting

My husband and I arrived in Texas Thursday evening. We're so excited to be back, but we can't wait to get our own apartment and get everything moved in... it's still in the to-be-determined-when, transition state.

We looked at apartments at the beginning of June (last time we were in town), and we went out to look again today. There are a few more that I'd like to take a look at, but we're narrowing things down... mostly me, since Jeremy isn't as picky about apartment stuff like I am. With my migraines, it matters what lights are around the apartment, which way the windows face, and random other tedious details.

So, when we were out checking out apartments today, there was one thing that was said by several different people showing us the apartment models that really bugged me. They asked where Jeremy works, and he told them. Then, they asked me where I work... so, I said, "I don't." To which I received this response: "Good for you (one lady then said/wrote that I'm a homemaker)." Are you kidding me?!?! Yea, 'good for me' that I'm unable to work. I know that it was one of those things that people say and that there was no way for them to know that I couldn't work, but THINK BEFORE YOU SPEAK, PLEASE. Some things sound so simple and neutral, but can be so hurtful.

Just because I'm at home / not working, doesn't mean that Jeremy gets to come home to a Susie Homemaker wife...



He comes home to a wife that is more like this...

Packing for Our Move

Packing to move is not the most exciting thing to do! I really don't like the process... it's just so depressing to see all of the pictures and decorations taken down and packed into boxes. We've actually gotten quite a bit packed, but it seems like it's never-ending.

I've actually done a pretty good job with pacing myself, though I've had either a bad headache or a migraine the past two days.

I'm so excited about moving back to Texas, but... stress is stress, regardless of whether it's positive or negative. And, stress equals more pain. Even great changes can lead to more pain.

Texas-Bound

I know I've been away from my blog for a while, but we have AMAZING news!!! Jeremy got a JOB in TEXAS! We're heading back to our HOME state... NEXT week!

We're packing like crazy people... I can already feel it bad in my back. I'm trying to PACE myself by taking plenty of breaks (and making sure I eat/drink)... but it's DIFFICULT, when the timelines is so short. We're only moving the 'essentials' right now (we'll stay with family, until we get an apartment in Texas). But, I want to get as much PACKED as possible, so that we'll be able to get moved quicker (once we get an apartment).

I can't even begin to explain how BLESSED and EXCITED we are! I've been having some lower pain days for the better part of the last week or so, which has also been such a BLESSING. We have A LOT of packing to do and stress to deal with, but we're FINALLY heading home... and starting a new chapter of our lives. :-)
